The Oldhill Practice

The Oldhill Practice is currently being looked after under temporary provision by the Partners of the Lawson Practice.

The Practice is managed clinically and administratively by the Partners of the Lawson Practice.  Dr. Deborah Colvin is the Executive Partner/Clinical director and Yvonne Pope is the Managing Partner. There is additional support from key members of the Lawson Practice team. Our expertise has been utilised to kick-start services which were previously unavailable and to improve and expand services offered to the patients in the time we have been looking after the practice.

 

The practice has now won the tender for the GPLHC.

 

As of March 2010, Oldhill Practice will open as Hackney’s only GP Led Health Centre. The GP led health centre will offer bookable and non-bookable GP and walk-in centre services to both registered and unregistered patients. The GPLHC will be open 8 till 8 every day of the week, 364 days of the year. We will provide minor illness services and some enhanced services for non-registered patients.

 

We will need to expand the practice team to include nurses skilled in triage as well as those with practice nursing skills. We also are expanding the reception team to develop the receptionist as a person with real skills in sign-posting patients to the most appropriate person to look after their needs in the practice. We need to expand the medical team also. The practice team will be involved in the development of new systems and protocols. Successful applicants will need to be able to work as part of a team and to motivate from within, to be able to work under pressure and to help build a successful, happy and effective practice.

 

The GPLHC will move in to the North East Hackney Resource Centre when that is built, becoming part of the hub for the North East Polyclinic. This move will mean the organisation will benefit from state of the art premises and the opportunity to be fully involved in the development of the polyclinic hub.

 

Currently the list size is almost 4200 and growing with a view to becoming 6000.

The Practice is fully computerised and uses EMIS as its clinical system. We use Choose & Book for referrals and have pathology links to the local hospital. We offer patients the opportunity to book appointments and order prescriptions on-line.

 

This practice has the potential for considerable development both clinically and administratively and the successful post-holders will have an exciting and challenging opportunity to be part of a team that will develop new services for patients.

History The practice itself is situated just off of Stamford Hill and works from premises at Oldhill Street.  There are two integrated, converted shops spread over 3 floors.  Refurbishment is about to be carried out. As mentioned above, the Lawson Practice has been running this practice for the PCT since September 2007 when the previous GP retired.

Profile There are a large number of Gujarati-speaking patients (40%), as well as an increasing number of Eastern European patients. 

 

Consultations rates are high and may require translation or advocacy services.

 

The practice population contains all social classes although it is predominantly a deprived inner city area with a high rate of morbidity.

Workload  

-  The extent of this disadvantage is reflected in the practice workload.    GP appointments are 12 minutes long and nurse appointments 15 minutes on average.

 

-    We are currently offering Enhanced Services including Diabetes, Sexual Health, IUCD, Smoking Cessation, and INR testing and are increasing this list to offer the full range available through the PCT.

 

-    We enjoy good relationships with the Locality Mental Health Team and have an in-house psychology service for children two days a week.

 

-    Health Visitors and our Practice Nurse run the Baby Clinics and offer immunisations whilst one of the doctors offers child health surveillance.  We also have good liaison with Social Services regarding child protection issues.

 

-    The practice runs separate hypertension, IHD and minor surgery clinics.  We have managed to achieve high immunisation and cervical cytology targets in the past 18 months.

 

-    The practice is fully computerised using EMIS.  We strive hard to achieve good quality data capture, mainly through the use of templates.  We feel we have benefited greatly both clinically and administratively by working towards consistent data entry.

 

-      As part of our interest in personal development, continuing education and personal development is supported for all clinical and non-clinical staff.

 

Meetings There are regular practice and clinical meetings held at the practice. Outside visitors are invited to attend these meetings and the clinical meetings are designed to support clinical professional development.

Future plans We intend to offer additional enhanced services.  We would like to increase the amount of near-patient services.

 

We are a group of people who are keen to use new technologies where they can assist in patient care and so are constantly reviewing what is new and what might help us.  

 

We are actively involved in practice based commissioning and can see the potential to improve the range of  services we can offer patients by shifting work from secondary to primary care. We have strong links with East London Integrated Care, our local PBC organisation.

 

We will start teaching GP registrars and medical students in the near future as one of the PCT aims is to develop this practice as a teaching centre. One of the partners at the Lawson Practice is Head of the Centre for Medical Education at Queen Mary and Westfield College and is keen to develop medical student teaching here.
